Clinical Pharmacist

Role Summary:

The Clinical Pharmacist assists in the development and execution of numerous initiatives that improve the quality or cost effectiveness of drug therapy in coordination with other care team members for Evernorth Care Group. This role supports Diabetes Medication Management, Anticoagulation Management and Medication Review Post Hospital Discharge.

Responsibilities:

  • Perform as a Clinical Pharmacist for the assigned healthcare centers.

  • Assist in the development, support and monitoring of disease management and drug utilization management programs.

  • Identify classes of drug therapy with potential for improvement in quality of care, increased efficiency, and improved cost-effectiveness.

  • Responsible for development and monitoring of internal detailing process.

  • Provider, patient and pharmacy education through newsletters, presentations/in-services and one-on-one contact.

  • Frequent one-on-one contact with providers to discuss quality of care, cost effectiveness and utilization management.

  • Serve as a clinical pharmacy resource for pharmacists and providers.

Minimum Qualifications:

  • Bachelor’s Degree in Pharmacy required; PharmD preferred

  • Pharmacy License active in State of Arizona required

  • Minimum of 2 years pharmacy experience with a current and exemplary body of clinical pharmacy knowledge

  • Excellent verbal communication skills with the ability to communicate and maintain effective interpersonal and working relationships with professional and support staff, providers and all customers.

  • Excellent analytical and problem-solving skills

  • Ability to effectively educate Providers and staff members

  • Proficient with Microsoft applications including Outlook, Word, Excel and PowerPoint.

Preferred Qualifications:

  • Minimum of 2 years of experience in Clinical Pharmacy or completion of a Clinical Pharmacy Residency program with a minimum of one year of experience in a managed care setting.

  • Additional credentials such as BCACP, BCPS, CACP and or CDE a plus

  • Previous experience/training in medication reconciliation and MTM services

  • Bilingual skills (English/Spanish) a plus

If you will be working at home occasionally or permanently, the internet connection must be obtained through a cable broadband or fiber optic internet service provider with speeds of at least 10Mbps download/5Mbps upload.
