Crossroads Hospice & Palliative Care Referral Coordinator in Oklahoma City, Oklahoma

Referral Coordinator Qualifications:

  • 1 year of hospice experience is preferred.

  • Experience in a medical office or healthcare setting is required.

  • Excellent time management skills, detail-oriented tendencies.

  • Must have computer training and Windows-based applications experience required.

  • Must have reliable personal transportation, a valid driver's license, and the ability to safely and personally operate a vehicle.

  • Strong understanding of electronic clinical record utilization.

  • Responsible for maintaining complete confidentiality of patients' records and all other confidential information that may be received during the course of patient care, including financial information and personnel records.

  • Ability to assign a patient to the appropriate team in coordination with the required team leader(s) after admission.

  • Receive and promptly process referrals and/or inquires for services provided by hospice.

  • Schedule with patient /family, a hospice representative to provide information on hospice care and determine the need for service.

  • Extensive knowledge of hospice care and services.

Schedule:  Monday to Friday 8:30 AM to 5:00 PM
