AdventHealth RN Navigator Cardiology in Ocala, Florida

GENERAL SUMMARY:

The Cardiovascular Clinical Nurse Navigator would work under the direction of the Manager of the CV Surgery Clinic to facilitate and provide a comprehensive access point for cardiovascular services to include cardiology, cardiothoracic surgery and vascular surgery for the West Florida Division of AdventHealth. The nurse navigator coordinates all services required to provide multidisciplinary, quality assured, appropriate case selection and cost-effective care for patients and families. The Clinical Nurse Navigator must work seamlessly with: Patient, Family, Referring Cardiologists, and Healthcare teams in both the Hospitals and CV Surgery Physician Offices. Upon receiving Cardiology phone calls from referring Cardiologists and/or patient, the Navigator is responsible for coordinating Cardiology patient access/scheduling within the health system including Hospitals and CV Surgery Physician Offices.

PRINCIPAL DUTIES AND JOB RESPONSIBILITIES :

· Develops, plans and implements evidenced-based measures that increases the quality of care for the cardiovascular service line and ensures compliance with all regulatory guidelines.

· Supports and assists with data collection for process and quality improvement while understanding the department performance trends and established standards.

· Keeps Manager and Director informed and seeks assistance and direction from appropriate individuals when confronted with unfamiliar procedures and techniques before implementing them to ensure delivery of safe and effective patient care.

· Coordinates patient education and care: serves as an ongoing resource for patients and families during their diagnosis and treatment period; communicates information regarding patient status and needs; collaborates with other providers regarding how to best meet needs and sequence care; assesses patients’ learning style and health literacy in order to provide education on the appropriate patient level.

· The Nurse Navigator provides a comprehensive access point for cardiovascular services to include cardiology, cardiothoracic surgery and vascular surgery for the West Florida Division of AdventHealth.

· Coordinates all services required to provide multidisciplinary, quality assured, appropriate case selection and cost-effective care for patients and families.

· Provides telephone and face-to-face consultation with patients and providers to answer pertinent questions throughout their hospital experience; provides educational information to patient/family and reviews information with patient at discharge and/or within 48 hours post-discharge. Communicates with patient and family 24-48 hours after discharge to clarify any outstanding concerns to ensure patient understands follow-up.

· Communicates and collaborates with multidisciplinary team to ensure high-quality patient care. Acts as a liaison to physicians’ offices regarding proper orders, medical records, consents and any other required documentation/criteria ensuring they are received at the time of patient care or transfer.

· Maintains a professional relationship with all customers.

· Completes assigned work comprehensively and in a timely manner.

· Facilitates “on time” scheduling with coordinators, supervisors, physicians and patients.

Qualifications

EDUCATION AND EXPERIENCE REQUIRED:

· Bachelor’s degree in Nursing

EDUCATION AND EXPERIENCE PREFERRED:

o Master’s degree in Nursing, Business Management or Healthcare Management

KNOWLEDGE AND SKILLS REQUIRED:

· Effective verbal and written communication skills

· Ability to read, write and explain moderately complex information with a great deal of attention to detail

· Ability to communicate effectively in a supportive manner to patients, family, staff and physicians

· Ability to explain information to patients and families in a way the patient understands, and/or explain departmental or hospital procedures to employees, visitors and patients

· Ability to observe assess, guide and evaluate others

· Ability to communicate timely and in an open and honest manner

· Ability to collect data, interpret findings, set priorities and carry out established plans

· Flexibility in prioritization; ability to multitask; critical-thinking/problem-solving skills

· Self-motivated with ability to work independently and in a group setting

· Strong organizational skills

· Knowledge of PACS, Heartlab, LumeDx, Cerner and associated systems

· Three years of experience in cardiac care

KNOWLEDGE AND SKILLS PREFERRED:

· Previous leadership experience (Preferred)

· Previous experience with projects or program start-up (Preferred)

· Computer skills such as Microsoft Word, Access, Excel and Outlook (Preferred)

LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:

  • Current valid State of Florida license as a registered nurse

  • Current valid BLS certification

LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED :

  • Current valid ACLS certification
